No Surprises Act / Good Faith Estimate Notice

Lifetouch LLC is a cash-pay practice. Under the No Surprises Act, uninsured and self-pay patients have the right to receive a Good Faith Estimate (GFE) of expected charges before scheduled services.

Your Right to a Good Faith Estimate

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services, including related costs like procedures, before you receive care. You can ask your provider, or any provider or facility you choose, for a Good Faith Estimate before you schedule an item or service, or at any time you request it.

If Your Bill Is Higher Than Expected

If you receive a bill that's at least $400 more than your Good Faith Estimate, you have the right to dispute the bill. For more information about your rights under the No Surprises Act, visit cms.gov/nosurprises or call 1-800-985-3059.
